All presenting whole blood, red cell
and platelet donors at Heartland centers and mobile blood
drives will be entered once in the drawing for each visit
between 12/21/09 – 1/6/10.

One winner chosen for each day of
Heartland’s blood collection (12/21/09 thru 1/6/2010)
excluding 12/25/09 and 1/1/2010 or any day in which zero
donors present to a Heartland center or mobile between
12/21/09 and 1/6/2010; ie: no winner if all Heartland
facilities are closed due to weather and no donors are
drawn.

Odds of winning are based on the
number of entrants.

Heartland Blood Centers reserves the
right to disqualify any entrant.

Heartland Blood Centers is not
responsible for damage to property once purchased.

Winners may purchase any item(s)
using gift card; not limited to purchase of TV.

Winner must show valid
identification to claim prize.

Prize is non-refundable and cannot
be combined with any other offer.

Heartland Blood Centers’ employees
or family members of Heartland Blood Centers’ employees are
not eligible to win.

Blood drive sponsors and/or blood
donors have the right to decline participation.

The names of the prize winners will
be announced and made available on our website
www.heartlandbc.org.

While winners will be notified
daily, the complete listing of all winners during the
promotion period will be posted to our website on Wednesday,
January 7, 2010, by close of business.

Prize winners will sign a statement
acknowledging responsibility for all associated taxes.
Prize winners will also sign a release, as a condition of
accepting the prize that his/her name, likeness, name of
community and testimonial may be used in Heartland publicity
of any type.

Prize winners, in signing the
above-mentioned release as a condition of accepting the
prize, will acknowledge and agree that it is
understood that no part of the prize is transferable to
other parties and that no part of the prize will be
converted to cash or other financial gain.

As a condition of accepting the
prize, winners must show valid ID and agree to provide
address and social security number in order for Heartland
Blood Centers to issue the winner an IRS Form 1099. Winner
will be responsible for all applicable taxes.

Must be 18 or
older to qualify.
